Síndrome do Túnel do Carpo Online (Carpal tunnel syndrome)

 

A síndrome do túnel do carpo (STC) é a neuropatia compressiva de ocorrência mais frequente. É decorrente da compressão do nervo mediano no punho. A história e o exame físico, incluindo sinais provocativos como os testes de Tinel e Phalen, foram considerados altamente sugestivos do diagnóstico. Os exames de eletroneuromiografia (EMG) são geralmente considerados para comprová‐la. Mais de 80% dos pacientes têm mais de 40 anos e as mulheres são mais comumente afetadas do que os homens. O envolvimento é bilateral em quase metade dos casos, mas a mão dominante é a primeira e mais gravemente acometida. Essa síndrome é considerada principalmente um distúrbio sensitivo, porque as fibras sensitivas do nervo mediano são mais afetadas do que as motoras. Assim, os pacientes queixam‐se de sintomas de STC, como dor leve e sensação de formigamento no polegar, indicador e dedo médio, ou parestesia e rigidez na mão, principalmente à noite. A atrofia da musculatura tenar, fraqueza ou falta de jeito da mão, pele seca, inchaço ou mudança na cor da mão também podem ser vistos em alguns casos; contudo, geralmente são achados tardios e esse estágio deve ser evitado pelo tratamento precoce e correto. A STC ocupacional é causa de absenteísmo no trabalho e de disputas legais. A síndrome do túnel do carpo é uma das doenças mais frequentemente tratadas pelos ortopedistas e especialistas em cirurgião da mão. Essa condição é responsável por substanciais custos anuais à sociedade, tanto em termos de perda de produtividade do paciente quanto em relação aos custos diretos do tratamento. Em muitos casos o tratamento conservador é ineficaz e há a necessidade de tratamento cirúrgico

Síndrome de Terson (Terson´s syndrome)

 

¨A síndrome de Terson é descrita como hemorragia vítrea secundária a hemorragia subaracnoidea ou subdural. O sangue pode estar localizado abaixo da membrana limitante interna da retina ou no espaço sub-hialoideo. A hemorragia intraocular tem sido atribuída ao rápido aumento da pressão venosa ou intracraniana, que rompe os capilares peri-papilares ou causa compressão da veia central da retina. Ocorre em cerca de 20% dos pacientes com hemorragia intracraniana e pressão intracraniana elevada, estando particularmente associada a hemorragia subaracnoide, devido a ruptura de aneurisma intracraniano. O prognóstico visual na síndrome de Terson é geralmente bom, com reabsorção espontânea do sangue. Ocasionalmente, resultados visuais ruins ocorrem por não absorção da hemorragia vítrea, dobras da retina, formação de membrana epirretiniana, alterações pigmentares da mácula, vitreorretinopatia proliferativa e atrofia óptica. Nos casos de hemorragia bilateral ou hemorragia vítrea de longa duração está indicada a vitrectomia¨
