Síndrome de Reiter Online (Reiter Syndrome)

 

¨A artrite reativa, antes chamada de síndrome de Reiter, é condição rara, de distribuição universal, caracterizada por uretrite, conjuntivite (ou, mais raramente, uveíte), lesões mucocutâneas e artrite asséptica. Acomete preferencialmente indivíduos adultos do sexo masculino. Caracteriza-se por poliartrite periférica soronegativa, com duração mais longa do que um mês, manifestando-se geralmente após quadro infeccioso disentérico ou urogenital. Acometimentos mucocutâneo, ungueal e ocular são comuns.  A síndrome de Reiter faz parte de um grupo de doenças denominadas espondiloartropatias soronegativas, que exibem predisposição hereditária associada à presença do antígeno do complexo de histocompatibilidade maior (HLA-B27) e têm em comum graus variáveis de manifestações artrocutâneas e oculares. Sua etiopatogenia não está completamente compreendida, porém a consistente associação com o HLA-B27.  Há duas formas epidemiológicas distintas: uma endêmica ou venérea, iniciada por uretrite inespecífica, cujo agente infeccioso mais importante é a Chlamydia trachomatis, sendo também implicados o Ureaplasma e o Mycoplasma como possíveis agentes; outra epidêmica ou pós-disentérica, na qual se inclui o paciente em questão, em seguimento após uma infecção intestinal por microorganismos Gram negativos chamados artritogênicos, incluindo os gêneros Shigella, Salmonella, Yersinia, Campylobacter e Clostridium. Não há cura para a síndrome de Reiter. Aproximadamente dois terços dos pacientes apresentam remissão do quadro em seis meses. Recorrências são freqüentes, com 40% dos pacientes apresentando um segundo episódio, como observado no paciente em questão. Na fase aguda, repouso e antiinflamatório não hormonal podem ser úteis.¨

Vitiligo Online (Vitiligo)

 

¨O vitiligo se caracteriza por manchas acrômicas relacionadas à perda de melanócitos da epiderme e folículo piloso, em qualquer área da pele, que não têm melanócitos. Incide em qualquer idade, em ambos os sexos e acomete até 2% da população em algumas regiões. Vitiligo é um transtorno de pigmentação freqüente na população mundial. Alguns fatores precipitantes para essa doença são: estresse físico e emocional, traumas mecânicos e substâncias químicas, como derivados do fenol. Doenças auto-imunes, principalmente as tireoidianas, podem estar associadas ao vitiligo. O vitiligo é dermatose de difícil tratamento e de significativo impacto psicossocial. Seu tratamento ainda oferece resultados limitados em alguns pacientes. Novas terapias têm sido propostas, como o uso de imunomoduladores tópicos, aliadas àquelas já consolidadas, como os psoralenos e os corticosteróides; o sucesso terapêutico, entretanto, está estritamente relacionado à qualidade da relação médico/paciente. Nos casos de vitiligo estável clinicamente, o transplante de melanócitos torna se uma opção terapêutica, sendo a técnica de enxertos autólogos por punch empregada com boa resposta na repigmentação. ¨
