Tosse Pós-Infecciosa em Adultos: FTTKA ( Posinfectious Cough in Adults)

– A tosse pós-infecciosa afeta 11% a 25% dos adultos após uma infecção respiratória. A tosse pós-infecciosa é definida como uma tosse subaguda, com sintomas que duram entre 3 e 8 semanas. A infecção precedente desencadeia uma cascata inflamatória, aumentando a sensibilidade brônquica e a produção de muco, ao mesmo tempo que reduz a eliminação de muco.

 

-O diagnóstico é por exclusão: este diagnóstico clínico requer infecção respiratória prévia, ausência de achados preocupantes no exame físico e exclusão de outros mimetizadores de tosse subaguda (por exemplo, asma, doença pulmonar obstrutiva crônica, doença do refluxo gastroesofágico ou uso de inibidor da enzima conversora de angiotensina). A coqueluche deve ser considerada em pacientes com tosse paroxística, vômitos pós-tosse e crise inspiratória.

 

– Os sinais de alerta e a duração prolongada da tosse devem levar a uma investigação mais aprofundada

 

– Nenhuma evidência apoia o tratamento farmacológico, que está associado a danos

 

– Tranquilizar os pacientes de que a tosse pós-infecciosa é limitada no tempo e a auto-resolução pode reduzir prescrições desnecessárias, inclusive de antibióticos.
