Pericardite Tuberculosa Online (Tuberculous Pericarditis)

 

¨- Uma mulher de 29 anos de idade apresentou-se ao pronto-socorro com uma história de 1 mês de agravamento da fadiga e fraqueza e uma história de 2 semanas de febre e dispnéia. Ela emigrou de Angola, para Portugal, 9 anos antes da apresentação. A temperatura era de 39,5 ° C, pressão arterial de 80/50 mm Hg, freqüência cardíaca de 135 batimentos por minuto e frequência respiratória de 32 respirações por minuto. No exame, presença de ingurgitamento veias do pescoço da paciente e os sons cardíacos estavam abafados. Um eletrocardiograma mostrava taquicardia sinusal e complexos de baixa voltagem em todas derivações. O ecocardiograma transtorácico revelou um grande derrame pericárdico com evidência de tamponamento. A pericardiocentese foi realizada e a estabilidade hemodinâmica foi obtida após aspiração direta de 1400 ml de líquido purulento; um dreno pericárdico foi colocado. O nível de adenosina desaminase no líquido pericárdico foi de 100 U por litro (faixa de referência, 0 a 9), e o exame do líquido pericárdico revelou bacilos álcool-ácido resistentes. Um diagnóstico de pericardite tuberculosa foi feito. O teste para o vírus da imunodeficiência humana foi positivo. O paciente foi tratado com isoniazida, rifampicina, pirazinamida e etambutol por 2 meses, seguido de rifampicina e isoniazida por mais 4 meses juntamente com prednisolona por 6 semanas para prevenção de pericardite constritiva. A terapia antiretroviral também foi iniciada. A febre desapareceu na primeira semana de tratamento.¨

¨- A tuberculose (TB) é uma doença infecciosa caracterizada pela ocorrência de inflamação granulomatosa com necrose, com localização predominante no tecido pulmonar. Brasil faz parte do grupo de 22 países que juntos são responsáveis por 90% dos casos de TB no mundo e, apesar da redução da incidência da doença no país, ainda constitui uma importante causa de mortalidade, com 52.199 óbitos registrados entre os anos de 2003 e 2013. A TB pode acometer o pericárdio e levar à uma condição clínica conhecida como pericardite tuberculosa (PT), que em sua apresentação típica se dá na forma de pericardite constrictiva (PC). A PT acomete entre 1% e 4% dos pacientes diagnosticados com TB, representa 10% de todos os casos de pericardite e sua taxa de mortalidade pode chegar a 90% quando sem o correto diagnóstico e tratamento. O quadro clínico da PT é variado e pode incluir: febre, dispneia, dor torácica, palpitação, ganho ou perda de peso, pulso paradoxal, turgência jugular, sinal de Kussmaul e atrito pericárdico.
O diagnóstico da PT é feito pela identificação do bacilo de Koch (BK) no líquido ou no tecido pericárdico. A pericardiocentese deve ser feita em todos os pacientes com suspeita de PT. A biópsia pericárdica realizada em conjunto leva ao diagnóstico mais precoce se comparado a realização de pericardiocentese isolada. A pericardite tuberculosa associada a HIV deve ser suspeitada em pacientes de alto risco (história de exposição, estado imunocomprometido). Em pacientes com quadro de pericardite em que as características clínicas e epidemiológicas aumentam a suspeita de pericardite por tuberculose, o tratamento pelo esquema RIPE associado a corticóides deve ser realizado mesmo que os exames não comprovem a presença do agente etiológico.¨
