Cardiomiopatia Diabética Online (Diabetic Cardiomyopathy)

 

¨Embora a cardiopatia isquêmica seja a principal causa de morte em pacientes diabéticos, a cardiomiopatia diabética (CMD) é cada vez mais reconhecida como uma entidade clinicamente relevante. Considerando que compreende uma variedade de mecanismos e efeitos sobre a função cardíaca, aumentando o risco de insuficiência cardíaca e agravando o prognóstico desta categoria de pacientes, a CMD representa uma importante complicação do diabetes mellitus, com um desenvolvimento silencioso em seus estágios iniciais, envolvendo complexas patofisiológicas. mecanismos, incluindo estresse oxidativo, manuseio defeituoso do cálcio, alteração da função mitocondrial, remodelamento da matriz extracelular e consequente deficiência da contratilidade dos cardiomiócitos. Embora a CMD seja comum em pacientes diabéticos assintomáticos, é freqüentemente subdiagnosticada, devido a poucas possibilidades diagnósticas em seus estágios iniciais. Além disso, como uma estratégia de prevenção e tratamento para melhorar o prognóstico da CMD ainda não foi estabelecida, é importante identificar pontos de referência fisiopatológicos claros, identificar as possibilidades diagnósticas disponíveis e identificar potenciais alvos terapêuticos. A cardiomiopatia diabética (CMD) é uma disfunção cardíaca que afeta aproximadamente 12% dos pacientes diabéticos, levando à insuficiência cardíaca e morte. No entanto, não existe uma metodologia eficiente e específica para o diagnóstico da CMD, possivelmente porque os mecanismos moleculares não estão totalmente elucidados, e permanece assintomática por muitos anos. Além disso, a CMD freqüentemente coexiste com outras comorbidades, como hipertensão, obesidade, dislipidemia e vasculopatias. Assim, a CMD humana não é especificamente identificada após a insuficiência cardíaca ser estabelecida. A ecocardiografia tem sido tradicionalmente considerada o exame de imagem padrão-ouro para avaliar a presença de disfunção cardíaca, embora outras técnicas possam cobrir a detecção precoce da CMD pela quantificação do metabolismo miocárdico alterado. Neste sentido, a ressonância magnética de fase e a ecocardiografia 2D / 3D-Speckle Tracking podem potencialmente diagnosticar e estratificar pacientes diabéticos. Além disso, essa informação poderia ser completada com uma quantificação de biomarcadores plasmáticos específicos relacionados aos estágios iniciais da doença.  Assim, sugere-se uma combinação de ferramentas de diagnóstico minimamente invasivas para o reconhecimento de CMD com base em técnicas de imagem e medições de biomarcadores plasmáticos relacionados. O tratamento da cardiomiopatia diabética não difere das miocardiopatias de outras etiololgias e deve seguir as diretrizes de acordo com o comprometimento da função ventricular. O tratamento é multifatorial (antidiabético, estatina, inibidores da enzima conversora da angiotensina ou antagonistas de receptores AT II, betabloqueadores, moduladores metabólicos, etc). ¨
