Cardiomiopatia & Síndrome de Takotsubo: Casos Clínicos Online (Cardiomyopathy & Takotsubo Syndrome: Case Reports Online)

¨Também denominada miocardiopatia de stress ou síndrome do coração-partido, cardiomiopatia de Takotsubo ou síndrome de balonamento apical. A cardiomiopatia de estresse/Takotsubo (CT) é uma síndrome cada vez mais relatada, caracterizada por disfunção sistólica transitória na ausência de doençca coronária obstrutiva, a qual imita o infarto do miocárdio. A exata incidência de CT é desconhecida, mas estudos recentes revelaram uma prevalência de aproximadamente 2% de pacientes com síndrome coronariana aguda nos Estados Unidos e Europa. Um estressor emocional ou físico frequentemente precede o desenvolvimento de sintomas, mas a fisiopatologia da CT é incerta. Ocorre habitualmente após episódio de stress físico ou emocional1 (embora esse antecedente não seja essencial ao diagnóstico). Várias hipóteses têm sido propostas, incluindo a cardiotoxicidade mediada por catecolaminas, espasmo da circulação coronária microvascular e disfunção das células endotelial. Atualmente não existe um tratamento padrão para a MT, sendo fundamental o diagnóstico diferencial com SCA. O tratamento conservador dirigido à disfunção sistólica ventricular esquerda parece ser adequado. Atendendo ao excesso de catecolaminas que está na origem dessa doença devem manter-se os β-bloqueadores e iniciar-se inibidores da enzima de conversão da angiotensina (IECAs), podendo ser descontinuados os agentes antiplaquetários e nitratos. A duração ideal do tratamento permanece desconhecida, mantendo-se até a recuperação da função sistólica, que leva habitualmente uma a quatro semanas. Apresenta na maioria dos casos um prognóstico favorável.¨
