Síndrome de Boerhaave Online (Boerhaave syndrome)

¨É a ruptura espontânea do esôfago. A ruptura esofágica é decorrente de um súbito aumento na pressão interna do esôfago precipitada durante o ato de vomitar, observada em 77% dos casos, como resultado de uma incoordenação neuromuscular do músculo cricofaríngeo. A síndrome de Boerhaave é uma doença de manifestação clínica variável, levando muitas vezes a um diagnóstico tardio e à sérias complicações. A apresentação clínica típica é de dor súbita, de localização torácica ou epigástrica, após episódio de vômito antecedido por um abuso alimentar e ou alcoólico. Normalmente está associada com enfisema cervicomediastinal e na forma da tríade de Mackler (vômito, dor torácica, enfisema subcutâneo). A hematêmese não é um sinal fidedigno de ruptura, pois foi demonstrada em apenas 30% de casos. A apresentação atípica e a raridade desta entidade, normalmente conduzem à demora no diagnóstico e requerem exames especializados para identificar a fístula esofágica. O diagnostico precoce aliado a um tratamento cirúrgico adequado pode diminuir as taxas de morbimortalidade desta importante afecção.¨
