Criptococose Online (Cryptococcosis)
¨Criptococose (torulose, blastomicose européia, doença de Busse-Buschke) é micose de natureza sistêmica de porta de entrada inalatória causada por fungos do complexo Cryptococcus neoformans, atualmente com duas espécies: Cryptococcus neoformans e Cryptococcus gattii. Ambas causam meningoencefalite, de evolução grave e fatal, acompanhada ou não, de lesão pulmonar evidente, fungemia e focos secundários para pele, ossos, rins, supra-renal, entre outros. A mortalidade por criptococose é estimada em 10% nos países desenvolvidos chegando a 43% nos paises em desenvolvimento. Cryptococcus neoformans é cosmopolita, ocorre em diversos substratos orgânicos, freqüentemente associa-se a habitat de aves, excretas secas, ricas em fontes de nitrogênio, como uréia e creatinina. Condições favoráveis ao crescimento abundante desta levedura formam microfocos, notadamente em centros urbanos e relacionados a pombos. O ambiente domiciliar, particularmente na poeira doméstica, pode apresentar positividade (50% e 13%). Cryptococcus gattii tem sido isolado do ambiente principalmente em regiões tropicais e subtropicais, mas áreas de clima temperado e frio devem ser incluídas em sua distribuição. A criptococose ocorre como primeira manifestação oportunista em cerca de 4,4% dos casos de aids no Brasil. A criptococose é uma das micoses de mais fácil diagnóstico por apresentar marcado tropismo neurológico, abundância de elementos fúngicos no líquor e nas lesões, presença de cápsula característica, diagnóstico imunológico e coloração tecidual específica. Existem várias formas clínicas e tratamento da criptocosose: comprometimentos pulmonar, neurológico, ósseo, ocular, cutânea, renal, incluindo prostatite criptocóccica.¨
