Choque Cardiogênico: Clínicos Online (Cardiogenic Shock: Case Reports)

Choque cardiogênico é uma condição clínica de inadequada perfusão tecidual devido à disfunção cardíaca. Sua definição inclui os seguintes parâmetros hemodinâmicos: hipotensão persistente (pressão arterial sistólica < 80 a 90 mmHg ou pressão arterial média 30 mmHg menor que a pressão arterial média basal), com acentuada redução no índice cardíaco (< 1,8 L/min por m², sem suporte hemodinâmico ou < 2,0 a 2,2 L/min por m² com suporte hemodinâmico) e pressões de enchimento ventricular normal ou elevadas. A etiologia mais comum do choque cardiogênico é o infarto agudo do miocárdio com supradesnivelamento do segmento ST (IMCSST), levando à insuficiência ventricular esquerda, mas também pode ser causado por complicações mecânicas, como insuficiência mitral aguda, ruptura do septo interventricular ou da parede livre do ventrículo esquerdo. No entanto, qualquer causa de disfunção ventricular aguda grave, esquerda ou direita, pode levar ao choque cardiogênico. As taxas de mortalidade ainda são altas, oscilando nos estudos mais recentes entre 42 e 48%.

Tenossinovite de Quervain Online (Quervain´s Tenosynovitis)

A tenossinovite de De Quervain (TDQ), decorrente da inflamação dos tendões dos músculos abdutor longo e extensor curto do polegar, vem aumentando sua incidência no mundo nos últimos anos, sendo mais uma dentre as doenças ocupacionais relacionadas ao trabalho. Seu tratamento clássico é realizado através de anti-inflamatórios não esteroides, infiltração de corticoide local, talas, fisioterapia e cirurgia reparadora. A acupuntura desponta como alternativa terapêutica válida e de baixo custo para tratamento dessa e de outras síndromes, dentro das práticas integrativas e complementares.
