Tumor Estromal Gastrointestinal (Gastrointestinal Stromal Tumor – GISTs)

 

O Tumor estromal gastrointestinal (Gastrointestinal stromal tumor – GIST) é a neoplasia mesenquimal mais comum do trato digestivo, possui comportamento biológico variado e a principal forma de tratamento é a ressecção cirúrgica.

 

– Tumores Estromais Gastrointestinais (GISTs) são raros tumores do trato gastrointestinal. No mundo, apresenta incidência 1,5 a 1/100.000/ano e, no Brasil, estima-se que existam 1.800 novos casos a cada ano. Apresenta maior frequência entre a faixa etária de 60 a 65 anos e localização mais comum no estômago (40-70%), seguido do intestino delgado (20-40%), do colo/reto (5%) e do esôfago (5%). A localização do tumor influencia o seu comportamento biológico, sendo os tumores gástricos os que apresentam melhor prognóstico que os outros sítios cometidos.

 

– Seu diagnóstico é baseado no quadro clínico, nas características morfológicas, mas, sobretudo, pela presença da proteína c-KIT (CD117) detectada por método imunoistoquímico. Na maioria das vezes, são assintomáticos, descobertos incidentalmente por exames de imagem.

 

– Os fatores prognósticos mais importantes são o tamanho do tumor e o índice mitótico.

 

– A ressecção cirúrgica é a terapia de escolha e o quimioterápico mesilato de imatinibe, que está indicado nos casos de irressecabilidade ou doença metastática.
